A Tribute gift In Honour or In Memory of, is a meaningful way to recognize someone special in your life or in the life of someone close to you.

An In Honour donation lets you acknowledge someone for a special occasion like a birthday, wedding or anniversary. An In Memory gift lets you dedicate a donation in memory of a loved one.

When we receive your donation, the Foundation will send a personalized acknowledgement card to the family or, to whomever you designate, to let them know that a donation has been made.


Monthly giving is a convenient way to support the AWHF. Monthly payments provide a steady and predictable source of income, allowing the Foundation to plan for the future. You can  make a monthly contribution online through CanadaHelps.org. Just click the link in the box to the right. Monthly donation giving can be cancelled at any time.


Every donation helps! You can make a single donation to the Foundation in several ways: online, by mail, or drop it off at either the Annapolis Community Health Centre Business Office or the Foundation Office on the lower level of the Health Centre.

The mandate of the Annapolis West Health Foundation  is to support health and wellness in the area served by the Annapolis Community Health Centre. The AWHF does this by raising funds to support programs and purchase equipment that are consistent with its mandate.

The Board of Directors and the staff of the AWHF are committed to protecting the privacy of our donors and potential donors.

Privacy practices are consistent with PIPEDA (Personal Information and Electronics Documents Act).

How We Acquire and Use Your Information

Information that you share with us when you make a donation to the AWHF such as, title, address, telephone number, email address, donations history and other relevant information is collected for the following purposes:

  • To comply with Canada Revenue Agency guidelines
  • To provide you with appropriate recognition
  • To provide you with information regarding AWHF activities
  • To provide you with further opportunities to support AWHF

Your Information Stays With Us

The information you provide stays with us in a password protected database or in a secured manual filing system within the AWHF office. All staff and AWHF Directors sign a confidentiality agreement as part of their terms of engagement with the AWHF.

The AWHF does not sell, lease, trade or share your information with anyone.

Donor Recognition

All donors will be offered the opportunity to have their donation recognized on the information board located in the lobby of ACHC. This recognition is not automatic and will occur upon the donor providing consent.
